Patients in the Post-Acute-to-Home (PATH®) program—a proven approach to rehabilitative care that bridges the gap between hospital and home-- receive rehabilitation therapy services provided by Aegis Therapies® and skilled nursing services that contribute to their recovery, under the supervision of a physician, registered nurse and other skilled professionals provided as a Medicare benefit.

A team of doctors, nurses, therapists and social workers tailors individual programs that are built to meet each patient’s needs while also caring for the whole person. Regular family conferences are held to expedite recovery and aid the transition to home.

In addition to the unique specialty programs Aegis offers, their on-site rehab support and expertise compliments and strengthens our PATH services.
